An easier poll on peer-review

[Update: survey fixed!]

Following feedback on the poll on peer-review I rolled out the other day, I've tried to design an easier poll. HERE is a new survey that I think readers should have a much easier time with. Although it has 20 items, the items are so simple that I think it should take readers just a couple of minutes to click through. Hopefully we'll get some better data out of this one--and, although it's only an informal poll, my hope is that its results will give us some good material to discuss moving forward!

Maybe its my browser or something, but after the first question I have only one option on question 2 "disagree" and no options for the subsequent questions.
